Arizona TPA + POA needed

Important
To effectively communicate with tax offices on your behalf, you must grant us Third Party Administrator access and/or Power of Attorney. Each tax office has its own authorization process, and may require different actions for you to take.

Required: Power of Attorney (POA) for the Arizona Department of Economic Security

  1. Download the Arizona DES POA found at the bottom of this page
  2. Complete all fields regarding your business information
  3. Owner/operator will need to sign at the bottom of the form
  4. Provide OnPay with the completed form

Third-Party Access (TPA) for the Arizona Department of Revenue

Once you have updated your AZ Payroll Settings in your OnPay account with your AZ withholding ID, submit a request asking us to submit an access request for your AZ Taxes online account. Once we have initiated the request, we will follow up with you to approve our access to your AZ Tax account.
