Grant access to these tax offices when requested by OnPay
You may be asked to take the following steps to allow OnPay to communicate with tax offices.
You may be asked to complete one or more of these forms:
- Power of Attorney for the New York Department of Labor
- EZRep for the New York Department of Transportation
- Power of Attorney for the New York Department of Transportation
- Click the link above for the requested form
- Complete all fields regarding your business information
- The owner/operator will need to sign the form
- Submit a request and attach a copy of your completed form(s)

Reminder
