About Form 8655 - Reporting Agent Authorization

The Form 8655 - Reporting Agent Authorization is a legal IRS document signed by an owner or corporate officer authorizing OnPay to file and deposit taxes on behalf of their company. Without a Form 8655 on file, OnPay cannot enroll you in an Electronic Federal Tax Payment System (EFTPS), or make calls to the IRS on your behalf. Failure to complete this form may result in the form's rejection by the IRS, and cause an undue delay in processing.

 

What you need to do

In step three of setting up OnPay, we make it easy by filling out Form 8655 for you. All you have to do is sign.
