Important
To effectively communicate with tax offices on your behalf, you must grant us Third Party Administrator access and/or Power of Attorney. Each tax office has its own authorization process, and may require different actions for you to take.
Third-Party Access (TPA) for the Louisiana Workforce Commission
- Log into your LAWATS account
- Once logged in, select Manage Power of Attorney under Employer Services
- Select Add POA
- Agency (TPA) Federal ID #: 35-2647026
- Authorization Type: Tax
- Agent Mailing Address: 675 Ponce De Leon Ave NE Ste W207, Atlanta, GA 30308
- Preferred Mailing Address: Select your company’s address
- Click Submit
- Select Link Agent Account
- Agent Access Code: QRMHBEXG
- Agency (TPA) Federal ID #: Enter 35-2647026
- Click Submit
Third-Party Access (TPA) for the Louisiana Department of Revenue
Provide OnPay with the following information so that we may submit the third-party access request:
- LA Withholding Tax ID (Example: 1234567890)
- Deposit frequency
- Zip Code as registered with LA TAP
- Last payment amount made for withholding tax
Submit a request to provide OnPay with your Withholding Tax ID, Zip Code, and last payment amount.
Power of Attorney (POA) for the Louisiana Department of Revenue
Download the Louisiana DOR POA and enter your business information in the following order:
- Company Name
- Company Address
- Federal Employment Identification Number (12-3456789) (not SSN)
- The Owner/operator will need to sign on page 2
