Grant access to these tax offices when requested by OnPay

You may be asked to take the following steps to allow OnPay to communicate with tax offices.

Power of Attorney: Department of Employment and Workforce

  1. Download the South Carolina DEW Power of Attorney form
  2. Complete the following information:
    • Part 1: Employer information
    • Part 4: Signature of Taxpayer
  3. Submit a request and attach a copy of your form

Third-Party Access: Department of Revenue

You may be asked to submit a request and provide the following information:

  • FEIN or SSN as on file with the Department of Revenue for your business
  • Business name as on file with the state
  • State Withholding File Number/Account Number
  • Letter ID from a recent letter received from the DOR

Power of Attorney: Department of Revenue

  1. Download the South Carolina DOR Power of Attorney form
  2. Complete the following information:
    • Part I, step 1: Taxpayer information
    • Part I, step 3: Years or Periods - Enter the year you began using OnPay
    • Part I, step 7: Signature of Taxpayer(s)
  3. Submit a request and attach a copy of your form

Reminder
