Why is this important?
OnPay works with government entities on your behalf, reporting new hires to state labor departments, calculating tax rates, paying taxes and insurance contributions, and filing financial reports. To do this effectively, you will need to authorize us to act on your behalf with tax offices for each state where your company has a tax obligation.
For example, if you operate in both West Virginia and Ohio, you must complete and return a power of attorney for both states.

Power of Attorney (POA) for the West Virginia State Tax Department
- Download the West Virginia DOT POA found at the bottom of this page
- Complete section 1 regarding your business information
- Sign and date at the bottom of 4B
- This form must be signed by a witness or printed and signed by a notary public before it can be returned to OnPay.
- Email a copy of your completed Power of Attorney form to hello@onpay.com
Unemployment Compensation Division
Third-Party Access (TPA) for the West Virginia Unemployment Compensation Division
For OnPay to file and pay your West Virginia unemployment taxes, we must have third-party access to your unemployment tax account. Please complete the steps below.

Log into your WorkForce West Virginia account.
- If you are not registered for an account, click "Create an account here" to set up login credentials.
- Go to the Home Page.
- Click on "Manage Users". (Towards the bottom of the page under Management Tools)
- Add "onpayinc" as a user with the role of "Admin"
Power of Attorney (POA) for the West Virginia Unemployment Compensation Division
- Download and print the West Virginia UCD POA found at the bottom of this page.
- Enter your company name, Employer Number, FEIN, and address in the top four spaces
- This form must be signed by a notary public and have a notary seal at the bottom of the second page before it can be returned to OnPay.
- Email a copy of your completed Power of Attorney form to hello@onpay.com
