Why is this important?
OnPay works with government entities on your behalf, reporting new hires to state labor departments, calculating tax rates, paying taxes and insurance contributions, and filing financial reports. To do this effectively, you will need to authorize us to act on your behalf with tax offices for each state where your company has a tax obligation.
For example, if you operate in both Ohio and Michigan, you must complete and return a power of attorney for both states.
Department of Taxation
Third-Party Access (TPA) for the Ohio Department of Taxation
To file your Ohio municipal and withholding taxes, you’ll need to grant OnPay third-party access to your Ohio Business Gateway account. To do this:
- Log in to the Ohio Business Gateway
- Access the “Account Access Management” page by clicking the “Lock” button in the top right-hand corner of the Gateway.
- Go to the “Account Access Requests” tab.
- Under "Requests for Account Access," locate the request from Stephanie Mendez, then click the grey check mark under "ACTIONS." (IMPORTANT: Approve only the request for FILING ADMINISTRATOR, not "Filer")
- Click the blue "Confirm" button.
- Please reply to the previous email informing OnPay that the request has been approved.
Power of Attorney (POA) for the Ohio Department of Taxation

- Complete Part 1 regarding taxpayer information
- Taxpayer will need to sign at the bottom of the form
- Email your completed Power of Attorney form hello@onpay.com
Department of Job and Family Services
Third-Party Access (TPA) for the Ohio Department of of Job and Family Services
Ohio requires mandatory online filing for Quarterly unemployment. Please complete the following instructions to add OnPay as your Third Party Administrator (TPA):
- Sign into your Ohio Source account.
- Navigate to the Manage My Account tab.
- In the Navigation Menu on the left, click Correspondence and Third Parties.
- Click Manage Service Functions.
- Do not select "Authorize Power of Attorney", as this is for a separate purpose in the ERIC system.
- Select the Tax: Reporting and Payments and Monetary Transactionsboxes so that OnPay can file on behalf of the employer.
- Click Manage Association in the bottom right corner.
- Click TPA Search.
- Use the company name OnPay and our account number 6000010814, then click Search.
- Once you’ve located our company, click Submit.
Power of Attorney (POA) for the Ohio Department of of Job and Family Services
- Download the Ohio JFS POA found at the bottom of this page
- Complete all fields in Section I regarding your business information
- Employer will need to sign in Section III of the form
