Submit a request and provide the following information:

• Business name as registered with the state of Vermont
• FEIN on file with the state of Vermont

Power of Attorney for the Vermont Department of Taxes

1. Complete the Vermont Power of Attorney form online 
2. You'll be prompted to verify your email address
3. Check your inbox for a new email from OnPay Documents, and click Review document
4. Complete all required fields marked with a red asterisk (*), then click Continue in the upper right
5. Click I agree in the upper right to submit the Power of Attorney form to OnPay
6. You'll receive an email copy of the completed form

Third-Party Access for the Vermont Department of Labor

Submit a request and provide the following information:

• State Unemployment Employer Account Number
• Password for your Vermont Internet Tax and Wage System account

Notary required for OnPay to file on your behalf 

Power of Attorney for the Vermont Department of Labor

1. Download the Vermont DOL Power of Attorney form
2. Complete boxes in the upper right with your business information
3. Enter your address and telephone number after "Client Address"
4. Print, sign and date the form 
5. A witness will need to sign the form
6. This form will also need to be signed by a notary public and have a notary seal before being mailed
7. Mail your completed, notarized form to the address listed at the top of the form
8. Provide OnPay with a copy of your completed form
