Submit a request and provide the following information:
- Business name as registered with the state of Vermont
- FEIN on file with the state of Vermont

Power of Attorney for the Vermont Department of Taxes
- Complete the Vermont Power of Attorney form online
- You'll be prompted to verify your email address
- Check your inbox for a new email from OnPay Documents, and click Review document
- Complete all required fields marked with a red asterisk (*), then click Continue in the upper right
- Click I agree in the upper right to submit the Power of Attorney form to OnPay
- You'll receive an email copy of the completed form
Required for OnPay to pay on your behalf

Submit a request and provide the following information:
- State Unemployment Employer Account Number
- Password for your Vermont Internet Tax and Wage System account

Notary required for OnPay to file on your behalf
Power of Attorney for the Vermont Department of Labor
- Download the Vermont DOL Power of Attorney form
- Complete boxes in the upper right with your business information
- Enter your address and telephone number after "Client Address"
- Print, sign and date the form
- A witness will need to sign the form
- This form will also need to be signed by a notary public and have a notary seal before being mailed
- Mail your completed, notarized form to the address listed at the top of the form
- Provide OnPay with a copy of your completed form

Can't access your OnPay account ?
Your password can be reset by clicking Forgot Password.
If you have Multi-Factor Authentication enabled for your account, but you don't have access to your authenticator, or are otherwise unable to access your account, call us at (877) 328-6505, between 9AM-8PM EST, Monday through Friday, for help.
As a reminder, never share your login credentials or MFA code with anyone. OnPay will never ask you for this information.
