Vermont TPA + POA needed

Important
To effectively communicate with tax offices on your behalf, you must grant us Third Party Administrator access and/or Power of Attorney. Each tax office has its own authorization process, and may require different actions for you to take.

Third-Party Access (TPA) for the Vermont Department of Taxes

For OnPay to file and pay your Vermont withholding taxes, we must have the information listed below. Submit a request and provide he following information about your company:

  • Business name as registered with the state of Vermont
  • FEIN on file with the state of Vermont

Once we have this information, OnPay can initiate the TPA request with the state.

Power of Attorney (POA) for the Vermont Department of Taxes

  1. Complete the Vermont POA using Dropbox Sign
  2. After clicking the link above, you'll be prompted to enter your email, then click Verify your email address
  3. Check your inbox for a new email from OnPay Documents, and click Review document
  4. Complete all required fields marked with a red asterisk (*), then click Continue in the upper right
  5. Click I agree in the upper right to submit the Power of Attorney form to OnPay
  6. You'll receive an email copy of the completed form

For OnPay to file and pay your Vermont unemployment taxes, we must have the items listed below. Submit a request and provide the following information:

Power of Attorney (POA) for the Vermont Department of Labor

  1. Download the Vermont DOL POA
  2. Complete boxes in the upper right with your business information
  3. Enter your address and telephone number after "Client Address"
  4. Print, sign and date the form 
  5. A witness will need to sign the form
  6. This form will also need to be signed by a notary public and have a notary seal at the bottom of the second page before being returned to OnPay
  7. Provide OnPay with a copy of your completed form
