Why is this important?
OnPay works with government entities on your behalf, reporting new hires to state labor departments, calculating tax rates, paying taxes and insurance contributions, and filing financial reports. To do this effectively, you will need to authorize us to act on your behalf with tax offices for each state where your company has a tax obligation.
For example, if you operate in both Vermont and New York, you must complete and return a power of attorney for both states.
Department of Labor
Third-Party Access (TPA) for the Vermont Department of Labor
For OnPay to file and pay your Vermont unemployment taxes, we must have the items listed below. Please email the following items to hello@onpay.com:
- State Unemployment Employer Account Number
- Password for your Vermont Internet Tax and Wage System account
Power of Attorney (POA) for the Vermont Department of Labor
- Download the VT DOL POA found at the bottom of this page
- Complete boxes in the upper right with your business information
- Enter your address and telephone number after "Client Address"
- Print, sign and date the form
- A witness will need to sign the form
- This form will also need to be signed by a notary public and have a notary seal at the bottom of the second page before being returned to OnPay
- Email a copy of your notarized Power of Attorney forms hello@onpay.com
Department of Taxes
Third-Party Access (TPA) for the Vermont Department of Taxes
For OnPay to file and pay your Vermont withholding taxes, we must have the items listed below. Please email the following items to hello@onpay.com:
- Business name as registered with the state of Vermont
- FEIN on file with the state of Vermont
Once we have both of these items, OnPay can initiate the TPA request with the state. We will respond to you with a form that you must sign and return to hello@onpay.com.
Power of Attorney (POA) for the Vermont Department of Taxes
- Download the VT DOT POA found at the bottom of this page
- Complete parts 1 and 2 of "Taxpayer" using employer/taxpayer information
- Signature of the employer/taxpayer is required in part 10 unless an authorized company representative is present (next)
- Signature of an authorized company representative can be used in part 11 instead of the employer/taxpayer
- This form will also need to be signed by a notary public and have a notary seal at the bottom of the second page before being returned to OnPay
- Email a copy of your notarized Power of Attorney forms hello@onpay.com
