Why is this important?

OnPay works with government entities on your behalf, reporting new hires to state labor departments, calculating tax rates, paying taxes and insurance contributions, and filing financial reports. To do this effectively, you will need to authorize us to act on your behalf with tax offices for each state where your company has a tax obligation.

For example, if you operate in both Washington and Oregon, you must complete and return a power of attorney for both states.

Power of Attorney (POA) for the Washington Employment Security Department

  1. Download the Washington ESD POA
  2. Complete all fields under "Employer Information"
  3. You must provide your UBI number in the provided field
  4. Sign at the bottom of the form, along with your name, title, and the date
  5. Provide OnPay with a copy of your completed form

Washington Labor & Industries

Third-party access (TPA) to your WA L&I account is recommended, to avoid unnecessary delays when working with tax offices on your behalf.

  • Submit a request and provide your UBI number, so that we may initiate a Third-Party Access request
  • We'll let you know once we've submitted our request for access
  • The account administrator for your company can log in and accept our request

Washington Paid Family Medical Leave

Power of Attorney (POA) for Washington Paid Family Medical Leave

  1. Download the Washington PFML POA
  2. Section One: Enter your Employer Information 
  3. Section Four: Enter the effective start date, then sign and date with your printed name and title
  4. Provide OnPay with a copy of your completed form

  • You must remove any agent currently listed on your WA PFML account before we can be added

    1. Call their Customer Care Team at (833) 717-2273
    2. Tell them you need to remove a "current employer agent"
    3. Confirm and note the agent's removal date
    4. Include the agent's removal date in the body of your email when returning your signed forms, or
    5. If you've already sent us your forms, submit a request and provide this removal date
