Illinois TPA and notarized Power of Attorney required

 Required for OnPay to pay on your behalf 

Third-Party Access the Illinois Department of Revenue

  1. Log In to your MyTax Illinois Account (you may need to register for an account)
  2. Click Manage My Profile, then click More...
  3. Click Manage Third Party Access, and select the following:
    • Allowed: Yes
    • Default Access: Third Party
    • Default Account Access: File & Pay

Once the above has been completed, submit a request for us to access your MyTax Illinois account, and provide the following information: 

  • 9-digit Account Zip Code
  • Last payment amount

If you cannot locate this information, contact the department by calling 800-732-8866.

Notary required for OnPay to file on your behalf 

Power of Attorney for the Illinois Department of Revenue

  1. Download and print the Illinois DOR Power of Attorney
  2. Complete all fields regarding your business information
  3. The taxpayer will need to sign at the bottom of the form, and it must be notarized
  4. Mail the notarized form to:

    ILLINOIS DEPARTMENT OF REVENUE
    PO BOX 19001
    SPRINGFIELD IL 62794-9001
  5. Submit a request and attach a copy of your form

Third-Party Access for the Illinois Department of Employment Security

  1. Log In to your MyTax Illinois Account. If you do not have an account with MyTax Illinois, you will need to register for one.
  2. Click View all account options hyperlink under "Unemployment Insurance".
  3. Click Add Agent under "Power of Attorney (POA)"
  4. Select Search by agent name
  5. In the field beside "Enter Agent Name", type "OnPay" and click Next
  6. Check the box marked "Select", and click Next
  7. Ensure that the following options are selected:
    1. Full access to view and manage
    2. UI Employer Contribution Report
  8. Click Next
  9. Check the box to "Agree", then click Submit

Reminder
OnPay also needs you to enter your state tax IDs and tax rates in order to properly calculate and remit all state taxes. See all Illinois tax guides→
