Montana TPA + POA needed

Important
To effectively communicate with tax offices on your behalf, you must grant us Third Party Administrator access and/or Power of Attorney. Each tax office has its own authorization process, and may require different actions for you to take.

Montana Department of Labor

  1. Download the Montana TPA
  2. Complete the "Employer" information section at the top 
  3. Add the signature of the Employer/Taxpayer
  4. Provide OnPay with a copy of your completed form

  1. Download the Montana POA
  2. Complete the following information in Part I:
    • Section 1: Taxpayer Information
    • Section 6: Signature of Taxpayer
  3. Provide OnPay with a copy of your completed form
