Colorado TPA needed

 Required for OnPay to pay on your behalf 

Third-Party Access for the Department of Labor and Employment

  1. Login to MyUI Employer+ 
  2. In the left menu of the employer homepage, click Account Maintenance (If you need to register, click Create an Employer User account first)
  3. Click Third Party Administrator (TPA) authorization
  4. Click New at the bottom of the page
  5. Under “Assign Third Party Administrator”, enter OnPay’s TPA ID: 500004356 d, click Next
  6. The start date is the first date of the quarter (1/1, 4/1, 7/1, or 10/1), leave "End Date" blank
  7. Assign TPA roles to your account - select each roll below, and  click Save
    • Edit Employment and Wage Detail
    • View Rate Notice and Voluntary Contribution
    • Edit Tax Payments Update and Submit

Third-Party Access for Colorado FAMLI

  1. Log into your Colorado FAMLI account (You may need to create an account
  2. Under "Manage My Accounts" select Manage Accounts
  3. You should see your company listed; click on the Account Name hyperlink
  4. Scroll down to "Third Party Administrator", and click Add
  5. In "Select Account", search for OnPay Tax Disbursement LLC
  6. The start date is the first date of the quarter (1/1, 4/1, 7/1, or 10/1), leave "End Date" blank
  7. Click Save
  8. Select Yes, add TPA
  9. You'll see OnPay Tax Disbursement listed under Third Party Administrator

Access you may need to grant upon request

You may be asked to grant this access in order for OnPay to work with tax offices on your behalf.

Third-Party Access for the Colorado Department of Revenue

  1. Sign in to your Revenue Online account
  2. Click Manage My Profile at the top of the screen
  3. Click More...
  4. Click Grant Third Party Access in the "Access" Box
  5. Click Next on the bottom right of the screen
  6. For "New Third Party Access", enter the following information: 
    • OnPay's username: OnPayTax
    • Set Access Level to ALL ACCESS
  7. Click Submit

Power of Attorney for the Colorado Department of Revenue

You can add a Power of Attorney online with the Colorado Department of Revenue, or by completing the attached DOR form.

  1. Download the Colorado DOR POA
  2. Complete all fields regarding your business information
  3. Owner/operator will need to sign at the bottom of the form
  4. Submit a request and provide a copy of the completed form

Power of Attorney for the Department of Labor and Employment

  1. Download the Colorado DOL POA
  2. Complete all fields regarding your business information
  3. Owner/operator will need to sign under "Employer Approval"
  4. Submit a request and provide a copy of the completed form
