Why is this important?
OnPay works with government entities on your behalf, reporting new hires to state labor departments, calculating tax rates, paying taxes and insurance contributions, and filing financial reports. To do this effectively, you will need to authorize us to act on your behalf with tax offices for each state where your company has a tax obligation.
For example, if you operate in both Arizona and New Mexico, you must complete and return a power of attorney for both states.

- Complete all fields regarding your business information
- Owner/operator will need to sign at the bottom of the form
- Email your completed Power of Attorney form hello@onpay.com
Arizona Department of Revenue
Third-Party Access (TPA) for the Arizona Department of Revenue
Email OnPay at hello@onpay.com once you have updated your AZ Payroll Settings in your OnPay account with your AZ withholding ID to request that we submit an access request for your AZ Taxes online account. Once we have initiated the request, we will follow up with you to approve our access to your AZ Tax account.
