 Required for OnPay to file on your behalf 

You must complete each of these forms:

  1. Click the links above to download both power of attorney forms
  2. Complete all fields regarding your business information
  3. The owner/operator will need to sign at the bottom of each form
  4. Submit a request and attach copies of your completed forms
