Required for OnPay to file on your behalf
You must complete each of these forms:
- Power of Attorney for the Arizona Department of Economic Security
- Power of Attorney for the Arizona Department of Revenue
- Click the links above to download both power of attorney forms
- Complete all fields regarding your business information
- The owner/operator will need to sign at the bottom of each form
- Submit a request and attach copies of your completed forms

Reminder
