Grant access to these tax offices when requested by OnPay
You may be asked to take the following steps to allow OnPay to communicate with tax offices.
You may be asked to complete one or both of these forms:
- Power of Attorney for the Arizona Department of Economic Security
- Power of Attorney for the Arizona Department of Revenue
- Click the link above for the requested form
- Complete all fields regarding your business information
- The owner/operator will need to sign at the bottom of the form
- Submit a request and attach a copy of your form(s)

Reminder
