California POA needed

Important
To effectively communicate with tax offices on your behalf, you must grant us Third Party Administrator access and/or Power of Attorney. Each tax office has its own authorization process, and may require different actions for you to take.

Complete the California EDD Power of Attorney Declaration (DE 48) online by logging into your Employer Services Online (ESO) account.

  1. Log into ESO account
  2. Go to Account Management
  3. Select Show More
  4. Select Power of Attorney

You'll need some information about OnPay to complete "Representative Designation" and "Authorized Acts" sections of the form.

Authorized Act 

  1. Select Specific Declaration
  2. Enter the date of your first payroll run in the field after "from"
  3. Leave the "To" field blank
  4. Select To represent the employer or taxpayer for any or all:
  5. Select Tax Reporting

Once you've completed all the required fields and added the signature of a corporate officer, partner, guardian, tax matters partner/person, etc., you can submit the form.
