Missouri Power of Attorney required

Power of Attorney for the Missouri Division of Employment Security

  1. Download the Missouri DES Power of Attorney form
  2. Complete the following information:
    • Section I: Business/Taxpayer Information
    • Section III: Signature of Business Representative/Taxpayer
  3. Submit a request and attach a copy of your notarized form

You may be asked to take these steps in order for OnPay to communicate with tax offices.

Power of Attorney for the Missouri Department of Revenue

  1. Download the Missouri DOR Power of Attorney form
  2. Complete the section at the top regarding your business information
  3. The employer will need to sign at the bottom of the form
  4. Submit a request and attach a copy of your notarized form
