Why is this important?

OnPay works with government entities on your behalf, reporting new hires to state labor departments, calculating tax rates, paying taxes and insurance contributions, and filing financial reports. To do this effectively, you will need to authorize us to act on your behalf with tax offices for each state where your company has a tax obligation.

For example, if you operate in both Kentucky and Tennessee, you must grant us access for both states.

Each tax office has its own authorization process, and may require different actions for you to take. We've detailed every step for each office below. We may need you to provide your tax registration information, or return a completed power of attorney (POA) form. In these cases, you'll see a button to "Submit a request". This request form is used to securely transmit your business information.

Submit a request and provide OnPay with one of the following:

  • Tax withholding amount from most recently filed return; or
  • Your PIN from your PIN letter

Don't have/can't find a PIN letter ?
Ask the state to send you a PIN letter through your MyTaxes account. The letter should arrive in the mail within 14 business days.

 

Department of Revenue

Power of Attorney (POA) for the Kentucky Department of Revenue

  1. Download the Kentucky 20A100 POA 
  2. Complete section 1 regarding your business information
  3. Enter your Kentucky Withholding Account Number in section 3
  4. Signature, printed name, date signed, and title of taxpayer in section 7
  5. Provide OnPay with a copy of your completed form

Office of Unemployment Insurance

Power of Attorney (POA) for the Kentucky Office of Employment and Training

  1. Download the Kentucky UI POA found at the bottom of this page
  2. Complete the top portion regarding your business information
  3. Signature, printed name, date signed, and title of taxpayer
  4. Taxpayer initials next to the statement reading "The legal mailing address of the named employer shall remain the same. The employer will continue to receive all correspondence pertaining to unemployment tax related matters."
  5. Provide OnPay with a copy of your completed form
