Indiana TPA and Power of Attorney required

 Required for OnPay to pay on your behalf 

Third-Party Access for the Indiana Department of Revenue

  1. If you haven't already, you'll need to create an INTIME account.
  2. Once your INTIME account has been created, submit a request and provide your account information so that we may initiate the request for TPA access.
  3. If your business is registered to a sole proprietor, we'll also need the owner's last name and SSN to request access.

Power of Attorney for the Indiana Department of Revenue

  1. Download the Indiana DOR Power of Attorney form
  2. Complete the following sections:
    • Section 1: Taxpayer Information
    • Section 2: Employer I.D. numbers and FEIN
    • Section 5: The taxpayer will need to sign at the bottom of the form
  3. Submit a request and attach a copy of your completed form

Third-Party Access for the Indiana Department of Labor

  1. Login to your Uplink ESS account and click Manage Users on the home page.
  2. At the bottom of the next page, there is a section labeled "External Account Authorizations (Agents)."
  3. Click Add Agent on the right.
  4. To add OnPay as an agent, search our Agent ID 114911.
  5. Select “OnPay Tax Disbursement LLC”, and click Next.
  6. Assign OnPay the Authorization Level of Administrator.
  7. Check the box under “Important Information” to confirm, then click Next.

Reminder

Forms you may need to complete upon request

You may be asked to take these steps in order for OnPay to communicate with tax offices.

  1. Download the IRS Form 2848 
  2. Complete the following sections:
    • Section 1: Your business information
    • Section 7: Signature of Taxpayer
  3. Submit a request and attach a copy of your completed form
