You may be asked to take the following steps to allow OnPay to communicate with tax offices.
Notary required
Power of Attorney for the Connecticut Department of Labor
- Download and print the Connecticut DOL Power of Attorney form
- Complete all fields regarding your business information
- The owner/operator will need to sign (notarized) at the bottom of the form
- The Power of Attorney Form must be notarized
- Mail the notarized copy to the address at the top of the form
- Submit a request and attach a copy of your form
Power of Attorney for the Connecticut Department of Revenue
- Download the Connecticut DOR Power of Attorney form
- Complete parts 1 and 2 regarding your business information
- The owner/operator will need to sign at the bottom of the form
- Provide your completed form to OnPay
