Withholding: Arizona A1-WP (Monthly Payment) and Arizona A1-QRT (Quarterly Return)
Unemployment: Arizona UC-018 (Contribution Report) and Arizona US-020 (Wage Report)
What information does OnPay need?
- Arizona Withholding Tax ID - Example: 12-3456789, the ID is the same as your Federal Employer Identification Number (FEIN).
- Arizona Department of Economic Security ID - Example: 1234567 8
- Arizona Unemployment Rate
What OnPay needs to electronically file AZ Withholding Tax:
Please log in using your Username and Password for AZ Department of Revenue Account.
- Once logged in, select "Security"
- Then select "Add User"
- Please use the following information:
- Full Name: Caroline White
- Email Address: carolinewhite@onpay.com
- Phone Number and Extension: 877-328-6505
- Once you submit our information, the Arizona Department of Revenue will send OnPay a password.
Where do I enter the information in OnPay?
- Login to OnPay
- Select the Settings menu icon
- Select Payroll Tax
- View/Update your Withholding Tax ID (in AZ this is the FEIN), Withholding Tax Deposit Schedule, Unemployment Tax ID, and Unemployment Rate (as a %).
I am an existing Arizona employer, where can I find this information?
If you are an employer with a history of payroll in Arizona you have likely obtained the IDs and online accounts necessary for OnPay to process your tax filings.

Withholding Tax Account Number: This ID is the same as your FEIN.
Arizona Department of Labor: (602)-542-3340
Unemployment Account Number: Your unemployment account number and tax rate on notices received from the Arizona Department of Economic Security.
Unemployment Tax Rate: This can be found on the top right of the Tax Rate Notice.
I am a new employer, do I need to register for Arizona Taxes?
- Do you have a business and/or employees residing in the State of Arizona?
- Have you processed payroll in OnPay and have received warnings stating we are missing tax information?
- Have you received an email from an OnPay Tax Agent requesting certain Arizona information?

Which Arizona agencies should I register for?
Arizona Department of Revenue: You will need to register with this department if you have employees that both live and work in Arizona, or for any employees that calculate Arizona withholding taxes regardless of where they live/work. if your business is located outside of Arizona, but your employees work or live in Arizona you will need to register with this agency.
Arizona Department of Economic Security: You will need to register with this department if you have employees that work in Arizona. If your business is located out of the state, but some of your employee's primary work location is in Arizona, you will need to register with this agency.
OnPay uses both the home address and the worksite assigned to each employee to decide which taxes should be calculated.
